Identification required to obtain a library card:

1) One of the following:

• Social Security Card
• Idaho driver's license number
• Idaho identification card number

We are aware that some individuals do not feel comfortable giving out their social security number, so we do provide the alternative of using a driver's license number or identification card number.

The driver's license facility has informed us that the second number listed on the license is simply a receipt number, and we cannot accept this instead of the actual driver's license number.

2) Picture ID - acceptable proof

• Valid driver’s license from any state
• Nampa Recreation Center card
• Valid state ID card
• Student ID card
• Company ID card
• Valid passport
• Current CostCo Card

3) Proof of residence - acceptable documents

• Valid Idaho driver’s license, issued within the last year
• Rental or lease agreement, signed within the last 2 months
• Proof of automobile insurance
• Bill or Business mail – must have name, address and date imprinted the contents within the last 2 months
• Forwarded piece of mail with official post office yellow sticker -- as long as the item is addressed to the current address and the date of the item is within the last 2 months
• Tax assessment notice
• Postcard sent by the library to your address
• Personal check imprinted with name and physical address

4) For Nonresidents only - Proof of City Property ownership
